This medical clearance form needs to be downloaded and taken to your athlete's physician for signature within the year of 2026.  No athlete will be considered fully registered until this form is completed and uploaded into thier Team Sideline Account. 

Use this form if your athlete is home schooled

This form is for a parent who wishes for thier athlete to come into our program from another towns boundries,  This is simply information for our president to review.   All decisons regarding boundry waivers will be decided solely by the NJAYF Executive Board of directors no earlier than July 15th.
